Submitting Nominations
Nominations for The Society of St. Luke can be made by an individual or an organization and should include the nomination form and/or a cover letter providing the following information:

  • A brief history of the nominee, including his or her accomplishments in the field of medicine. Please include an explanation of what uniquely qualifies the candidate for consideration. This history should include examples of community service.

  • Any supporting materials that illustrate activities, programs, or services and their results may be attached to the cover letter and/or nomination form.

  • Download and complete a nomination form. Nomination forms can be found on displays throughout the hospital or you can also call 803.256.5460 to request one be sent to you.

    Please be sure to review the membership criteria prior to submitting your nomination. Cover letters and nomination forms with supporting materials should be sent to:

    A letter of confirmation will be sent to all who submit a nomination. Nominators will also receive notification of the judge’s final decisions and an invitation to the awards ceremony in the Spring.
