Patient Information:

Admission Information
We want your admission and registration process to go as smoothly as possible. We have created the following guide to help ease the process.

Please bring the following items with you at the time of registration:

If you are being admitted, please leave all of your valuables at home for safekeeping.

Upon admission, you will be required to sign the following forms and authorizations (if you are over 18 years of age):

  • Consent and Conditions of Treatment
  • Privacy Practice Notification
  • Other payer and payment-related forms
Depending on your insurance carrier, you may be required to sign additional forms.

We kindly request that all co-pays, deductibles and out-of-pocket money due is paid at the time of admission for elective procedures. If you are unable to pay your portion of the bill, please let the associate know so that other options such as a payment plan or financial assistance may be discussed.

If you know your impending date of admission, you are encouraged to pre-admit. Preadmission will allow for some of the paperwork to be processed prior to your actual date of admission.

Please be sure to check with your health insurance provider for any specific authorizations or pre-certifications that may be necessary before admission. Often, if these incidentals are not handled up front, a financial penalty to the patient can result. Because there are several insurance companies, all with their own specific requirements, it is wise to check this information on your own. The hospital staff may not be familiar with your carrier and its requirements.

The Admitting Office for Providence Orthopaedic & Neuro Spine Institute is located just off to the left as you enter the main hospital lobby and is open from 5:45 a.m. until 4:30 p.m. After 4:40 p.m., the Emergency department registration associates handle all admissions and registrations. There is always someone to assist you 24 hours a day, seven days a week. Should you have any questions regarding the admission or registration process, please call us at (803) 856-4970. Please note that your bill does not include physician fees such as those from a surgeon, anesthesiologist, radiologist or pathologist. Diagnostic and therapeutic services provided by physician and physician groups are billed separately. These bills will come from the physician, not from Providence.

Chapel
Providence believes that healing is of the whole person: body, mind, feelings and spirit. The Pastoral Care staff brings you and your family the support, comfort, presence, prayer and counseling that help the healing process. Every morning and evening, a prayer is shared with the staff and patients over the hospitals’ speaker system. A chaplain is available for Providence patients and their families 24 hours a day.

The Downtown Campus chapel is located on the first floor next to the Prayer Garden. It is open to the public during hospital visiting hours. Mass is offered there Monday, Wednesday, and Friday at 11 a.m. You may view the hospital’s live mass from your room by tuning to Channel 13 Monday, Wednesday, and Friday at 11 a.m. Channel 13 also broadcasts relaxation music and hymns. Please contact the Pastoral Care office at 256-5305 for more information.

The Northeast Campus chapel is located on the first floor across from the cafeteria. It also is open to the public during hospital visiting hours. Mass is offered there Tuesday and Thursday at 11:30 a.m. Channel 13 is the Pastoral Care closed circuit television channel on that campus. Providence Hospital Northeast Pastoral Care office can be reached by calling 865-4516.

Checkout Time
When it is time for you to go home, a hospital representative will escort you to whatever transportation you have waiting at the main entrance.

Communication Assistance
Communication assistance is available upon request, including:

Text Telephone (TT)
Amplified telephone hand sets
Closed caption service

Please make all requests to your nurse.

Discharge Instructions
Instructions will be given to you before you leave the hospital. These instructions may include information about your medication, diet and a return appointment to see your physician. If home medical equipment or home health care is ordered by your physician, the discharge planner may coordinate this for you.

Flowers
Flowers are delivered to patient rooms Monday through Friday. Patients in the intensive care unit cannot receive flowers. If you have flowers delivered to you while you are a patient on this unit, we will ask the delivery person to return them to the florist. Deliveries may be received after patients transfer out of the intensive care unit.

Gift Shop
The Providence Orthopaedic & Neuro Spine Institute gift shop, located on the first floor, sells gifts items, magazines, candy, flowers, greeting cards, toiletries, flowers, plants and other personal items. Our gift shop is operated by the hospital's volunteers. The gift shop is open Monday through Friday from 9 a.m. until 5 p.m. Weekend and additional hours are subject to volunteer availability.

Insurance
Providence Orthopaedic & Neuro Spine Institute accepts the policies of health insurance companies and bills them for your inpatient, outpatient or emergency room services. Generally, insurance group plans, Medicare, Tri-Care Standard, HMOs (Health Maintenance Organizations), PPOs (Preferred Provider Organizations) and other such plans pay most of your bill. What they don't pay is classified as a deductible, co-insurance or both. You are responsible for any unpaid balance of your bill.

Individual plans and other insurance plans may pay a certain amount per day, which may be significantly less than billed charges. These policies are usually used to pay deductibles and co-insurance of group plans or to help pay for other medical costs. Internet Access
Wireless Internet access is available for your convenience in the cafeteria, freely available for use by patients, family members and other visitors.

Interpreters
The hospital has access to interpreters for a number of foreign languages should you require it. If assistance is needed, please ask your nurse.

Mail
Letters and packages for patients are delivered to your room Monday through Friday. Letters and parcels that arrive after you have gone home will be forwarded to the home address indicated on your records. Stamps and stationery are available in the gift shop. A United States Postal Service mailbox is located outside the main entrance of the hospital.

Medical Records
Your privacy is very important to Providence, but when the need arises, you may request a copy of your medical records by calling the Release of Information office at (803) 256-5722 on weekdays between 8:30 a.m. and 3:30 p.m.

A copy of a patient's Protected Health Information will be released upon completion of the following:

1. The requestor provides the following information on the appropriate form, available from the Release of Information office:

2. The requestor must present a picture ID before information is released.

3. The requestor shows under what authority he or she is receiving the requested protected health information. For example, a requestor must prove that he or she is the patient, or that he or she has legal documentation giving them rights to that patient's protected health information. Please note that these instructions do not pertain to health care providers.

Medication Form

The Universal Medication Form has a chart to help you and your family members keep a current record of your medications and health information. You should list all immunizations, allergies, prescriptions, and any vitamins, herbal supplements or over-the-counter medicines that you use. Take this form with you when you go to the doctor, pharmacy, hospital and dentist. The form can be downloaded and printed out.

Nutrition Services
Wholesome, nourishing and well-balanced meals are an important factor in your treatment and recovery. Providence Orthopaedic & Neuro Spine Institute makes every effort to provide nutritious meals that are prepared according to your doctor's orders. Your breakfast tray will include a menu from which you can order your meals. Please do not leave your menu on your breakfast tray. If your physician orders a diet consultation, a registered dietitian will be available to talk with you.

Proper nutrition is an important factor in your recovery. You should not eat any food brought to you unless your physician or nurse gives you permission. If you have any questions about your meals or diet, ask your nurse. Guest trays are provided at a nominal charge. Orders for guest trays should be placed at the nursing station at least two hours prior to the next meal. Gourmet meals are also available for you and your visitors at an additional charge.

Occasionally your meal may be delayed if you are scheduled for a special test or treatment. Your meal will be served after your examination or test or you may request a snack by notifying your nurse.

Parking
Parking for patients and visitors is available in the parking lot located across from the front entrance of the hospital. Patients and visitors are cautioned not to park in reserved areas or certain designated areas. Please be sure to lock your car. If you need assistance, please call the Safety and Security Department at 865-4690.

Personal Equipment
The only AC-operated equipment permitted in a patient's room is personal grooming equipment such as blow dryers, curling irons and razors. These articles must be in safe working condition. Prior to the use of these items please give them to the nursing staff so they can be inspected.

Pre-Surgery Information
After you are scheduled for surgery by your physician, you will receive a call from our Patient/Guest Relations department. You will be asked a series of questions, and will be pre-registered for surgery. At that time, our Patient/Guest Relations department will connect you to the pre-op nurse so that we can gather the necessary clinical information to determine if you will need to come in for any pre-op testing. If you need to come in prior to surgery, the pre-op nurse can schedule you for an appointment. If you do NOT need pre-op testing, you will arrive at the hospital on the day of surgery and can finish any paperwork with Admissions that is needed.

Providence Hospitalists Group
Your physician may request that your follow-up care in the hospital be managed by physicians from the Providence Hospitalists Group. Upon discharge, you will be returned to the care of your private physician. Members of the Providence Hospitalists Group are Laurie Milatz, DO; Dana Nairn, MD; Yohannes Negash, MD; and Mulugeta Tarekegne, MD.

Safety
Notice to Providence Patients
Providence is accredited by the Joint Commission on Accreditation of Healthcare Facilities. If you have concerns or questions about the safety or quality of care provided at Providence Hospitals, please contact the Joint Commission on Accreditation of Healthcare Facilities.

Smoking
For the benefit of others and for your health, Providence Orthopaedic & Neuro Spine Institute strictly prohibits smoking throughout the hospital. A designated smoking area is located between the main entrance and the medical office building. Please call Safety and Security at 865-4690 or ask your nursing staff for assistance. We thank you for your support in helping us provide a positive and healthy environment for all of our patients, visitors and employees.

Telephones and Computers
A telephone is located at every bedside in non-critical units and is free to all patients. Outgoing calls can be made 24 hours a day. Telephones are also located in the waiting rooms. We ask that you limit the use of these phones whenever possible. When dialing within the hospital, dial the last four digits of the number you are trying to reach. To dial outside the hospital, dial 9 and your number. For long distance calls, dial 9, 0, your number plus your calling card number, or call collect by dialing 9, 0 and the phone number. For assistance, dial 0. Pay telephones are located on the first floor across from the cafeteria, in the Emergency Room waiting area, and on each floor. Cellular phones are permitted within the hospital unless otherwise instructed by your health care provider. Internet connection is available for your computer by request. Please ask your nurse if this is a service you will need during your stay.

Valuable or Lost Items
Please do not bring items of value to the hospital. Providence will only be responsible for those valuables which have been specifically entrusted to the care of Providence Hospitals to be protected in its safe. You will be given a written receipt for all items, which must be presented when the items are withdrawn. All valuables that remain in your possession are exclusively your responsibility upon admission.

Your Hospital Bill
Payment for your services at Providence Orthopaedic & Neuro Spine Institute is due when you are ready to leave. Payment may be made at any registration site, located on the first floor of the hospital. Also, for your convenience, our Patient Financial Services department is located at 110 Gateway Corporate Boulevard in Suite 200, which is adjacent to the hospital. In addition, you can choose the convenience of mailing your payment. Included on your bill will be services you received from the hospital such as medications, medical supplies and other ancillary charges for tests. Please note that your bill will not include physician fees such as those from the surgeon, anesthesiologist, radiologist or pathologist. Diagnostic and therapeutic services provided by physicians or physician groups will be billed separately. These bills will come from your physician, not Providence Orthopaedic & Neuro Spine Institute. If you have any questions about these bills, please call the number printed on the statement you receive from them. If your insurance coverage does not pay all or part of your bill, please call your carrier for an explanation. To provide insurance information or to make arrangements for the balance due, you should contact our Patient Financial Services department at (803) 865-4700 Monday through Friday from 8 a.m. to 5 p.m.

Your Room
Your room assignment at Providence Orthopaedic & Neuro Spine Institute is based upon your admitting diagnosis and the bed availability on the day of your admission. All rooms in our Family Care and Maternity Care units are private (single bed). All rooms in our Transitional Care Unit are semi-private (two beds).
